After two back to back spinal surgeries I went to the Mosaic Senior Life center for rehabilitation. The care, professional services, and in particular the CRA services were beyond my expectations. The building and rooms are immaculate and management is always present. Peter Willner

An 82 year old nursing home resident suffering with Alzheimer’s Dementia was kept in the same diaper for 23.5 hours beginning on Wednesday, April 17.

Due to her illness, the resident cannot perform ADLs (Activities of Daily Living) and is completely reliant on care staff including nurses and CNAs (Certified Nursing Assistants).

At 7:51 p.m. on Wednesday, the resident was readied for bed after her weekly shower. By 7:16 p.m. the following evening the resident was still wearing the same diaper.

Upon reviewing video recordings from the in-room camera, the daughter contacted the on-duty nurse to inquire about care status. The nurse claimed that the resident had refused care. Additional review of camera footage revealed that the resident was not offered care by the on-duty CNA — even after the daughter expressed concern. It wasn’t until the daughter informed the nurse that the resident hadn’t been changed in nearly 24 hours that the nurse instructed the CNA to change the resident.

this place is so unprofessional! they had CNA training posted on the website for about a month and every time i called to get in contact with the hiring manager no one would answer. I even got a hold of her personal number and she didn’t didn’t pick up not once! don’t come here you’ll waste your time.

A very caring staff and excellent for rehab and hospice. My only complaint is that not all home care workers are acceptable.

The quality of the care and staff was mostly subpar. A care plan was never done on my Mom. I witnessed an aid yelling at a patient, when I reported it nothing was done. I asked to have my Mom moved, I was told there was no rooms available, when I said I was leaving they found a room. Concerns went unanswered. They were understaffed and it showed in the care and cleanliness. When I discuss this with the administrator, I was told they were not understaffed but they were raising the monthly fee $1000 to $19,500. Don’t be fooled by the marketing and new building. Since leaving I have heard the same concerns from other caregivers. The residents are afraid to complain because they’re afraid of the backlash.

The Jewish home is a beautiful state of the art facility. Inside there are individual houses and all private rooms to give residents a more home like experience. The nurses and CNA’s are the best and show love and compassion to everyone in their care. The entertainment is top notch , broadway style and the food is delicious . I could not find a better nursing home for my mom anywhere!

They are fabulous when you are private pay, but beware when your money runs out and you go on Medicare, game over!

My husband , Gerald Ohara was, admitted 10/1/14 to K3 with Alzheimers' The physical custodial care was very good as was the medical care. Gerry's AD progressed rapidly to paranoia, and I experienced the psychiatric services to be woefully inadequate. The psychiatrist is only on site one day a week. The APRN only there 2 days a week, and cannot be contacted after she leaves. When Gerry's symptoms excavated the APRN wanted to check his urine to rule out a urinary tract infection which can cause increased psych. symptoms. Her order was written on a Monday, and never executed. Gerry's paranoia got worse, by Thurs. of that week I had to get the sample. It was negative. Nursing did not follow up getting the urine. When questioned I was told, "we left the urinal in his bath room." With no APRN on site, watching my husband rapidly deteriorating I was livid. A Dr. came to see me on a Sat; she did nothing but talk me into waiting before an antipsychotic was prescribed. On that Sunday, I was called at home that Gerry Had broken the glass of the fire alarm, was bleeding and had to be transferred to Bpt. Hosp. Geriatric Psych where he died on 12/10/14.

I am writing this as a service to families who have a loved one with psych. issues. I think they need to advocate for 24/7 coverage of, at least, an aprn. I called the administrator of K3, who responded with, "that's how it is in all nursing homes; to which I responded, "isn't that sad." I have a nurse friend who worked, at one time in another facility. She, in tandem with a psychiatrist, and had a 24/7 on call system. It's only common sense.

I am following this up with a letter to the State of Ct., dept. of health.
