My mom has been at The Commons of Providence Assisted Living facility for nine months. The only way to describe her lifestyle is "flourishing!" This entire facility is top-notch. The caring and compassionate nursing, administrative, and activities staff, to the well-maintained buildings and grounds, to the meticulous housekeeping staff, to the delicious food and serving staff, (and anyone in between) - every facet is first class. We have been awed by the communication and underlying sense of cooperation between all areas of staffing. Never once have we heard "that's not my job" uttered! The staff not only cares for the residents, but there seems to be a genuine caring for one another. The residents are treated like family, with love and understanding, and we feel beyond blessed that we have chosen such an amazing facility.

My sisters and I brought our mom to the Commons of Providence the first part of January. We could not be happier with the excellent care they're giving her. There are so many activities and they include her in everything. She is fitting in so nicely and making new friends. If you're looking for a care facility for your love one I highly recommend the Commons of Providence.

3 yrs ago today my mom's life and the lives of my dad, their children & grandchildren took a drastic & devastating change which resulted in her death one week later. My mom had been a resident in the memory care unit for less than 30 days. My mom was not at the end-of-life trajectory for her dementia diagnosis. I got a call informing me over two hours after a situation in which I was told my "mom had been involved in an altercation with another resident at 6:05 a.m." and that "she had fallen on her butt". (Later in inquiring about the situation and circumstances, I learned this was NOT witnessed by any staff member nor could they verify it as the altercation supposedly occurred in an area of their facility where the video recording device used for surveillance had a known "blind spot"). I was told her vital signs during the call and alarmingly her oxygen saturation was 85% on room air (normal is at least 92% or higher) and it was measured at 97% the day before during a preoperative evaluation. I do not know about you, but I have fallen on my butt before and have NEVER experienced four fractured ribs in my posterior upper back near my spine with one being broken in TWO different areas and puncturing a lung causing it to collapse!!! OH law requires an assisted living/residential care facility (RCF) to notify the 'sponsor' and the attending physician or their designee within ONE hour of a change in a resident's condition. This did not happen. In addition, the resident is supposed to be assessed by a RN after a fall; this also did not occur. The fact is my mom's attending physician (who was also the medical director of this RCF) and the advance practice RN (nurse practitioner whom the physician employed) to whom he had delegated oversight of the residents' care and treatments were NEVER notified by a RCF staff member of this incident and my mom's change in status. In speaking in person to the pulmonologist who cared for and treated my mom, I asked him if the injuries she sustained were consistent in his professional opinion and experience with someone who had supposedly "fell on their butt" and he nodded no and stated "your mom was the victim of blunt force trauma". My mom died as a result of care (actually the lack there of) that was far below recognized expectations and standards of care at the hands of several personnel at this facility. I implore you to do due diligence before deciding to place a loved one at this facility. A former administrator at this RCF went so far while I was seeking information about events concerning this situation involving my mom as to try to have the ombudsman assigned to this RCF terminated from her role and responsibilities!!! Nothing will bring my mom back from her untimely death. As a RN who has practiced for 42 years, I hope and pray that your loved one and family do not experience the pain, suffering, emotional turmoil, and loss of your loved one at the hands of incompetence and acts of omissions as my mother and family did first hand at this facility and their cavalier personnel. God rest her soul as I hope she is resting in peace in his heavenly kingdom as she did not die from natural causes, nor did she deserve to die this way!

Mom's three years at TCOP assisted living were great. Even thru the pandemic the staff went out of their way to make the residents stay even better. But just day to day the staff is amazing and that includes every member. The care, concern and compassion given is exceptional. TCOP is definitely a place to check out for your loved ones.

I took care of my mother for 10 years she had Alzheimer disease.After she passed I started working at The Commons of Providence Assisted living. It was like my calling to take care of elderly people. If I would have known how much the staff cares about the residents and the way they treat them as if they were family I would have placed my mom here. I have been working with this company for 24 years. And this is where Home is here. Thank- you for you have done for me. 🙂
